Guildlines and Limitations
THIS IS NOT AN INSURANCE PLAN, this is a discounted
dental plan and can not be used in conjunction with
another dental plan.

Payment of full plan fee is made in advance.

All fees for any needed dental work is to be paid in full
at the time of service. Financial and payment options
are available but at full usual and customary fee.

No refunds of plan fee will be issued at any time if
participant decides not to use the plan.

The plan does not cover referrals to specialists.
This plan covers only care provided by Dr. Sulfaro
